Medical renewals usually are available 2—3 months prior to your renewal date.  Ancillary benefit renewals usually become available 45—60 days prior to your renewal. In some circumstances they will be available later than that.

If you have not received your insurance card yet & think you should please call us. Most cards come 7-10 business days from the time the enrollment was processed. Depending on the time of year & carrier, enrollments get processed anywhere from a couple of days to a month. Ask your physician if you can fax or send them a copy of your ID card once you receive it. Most physicians will understand.  You can also give out our number (616) 301-3060 so your physician can verify that you are in the process of being enrolled.

What do I do if  I haven’t received my insurance card yet?

What is considered a qualified expense with my H S A?

You may only add dependents at a time other than open enrollment if a qualifying event has happened. You must add the dependent within 30 days of the event.

 

When can I add other family members to my coverage?

Please refer to your specific employee handbook for specifications on waiting periods & eligibility. If you are eligible you have 30 days (before/after) your effective date to submit all enrollment information needed. If you have not just become newly eligible you may enroll in coverage at open enrollment or within 30 days of a qualifying event.

When can I add coverage for myself?

¨ Marriage/Divorce/Death of Spouse/legal separation/annulment

¨ Birth/Adoption/Placement for Adoption/Death of Dependent 

¨ Change in employment status of employee, spouse, or dependent (spouse terminates from employment)

What are Qualifying Events?

If you have recently lost coverage through your employer you may have the option to elect COBRA coverage if your employer offers COBRA. If your employer does not offer COBRA coverage you can look into individual coverage options or group conversion options.
